SummaryThe TAG score sheet is a 1 page, 7 tick standardised assessment of the severity of a patient’s mental health problems. It involves the GP and other referrers, or mental health professionals, rating the severity of the patient’s difficulties in three areas: a) SAFETY assesses the level of concern about intentional self-harm (domain 1) and unintentional self-harm (domain 2) As well as the TAG score sheet, two further pages are available. An evidence-based checklist offers guidance on the issues to consider when assessing each domain, and further guidance provides more details on administration and use of the TAG. However, these are optional - the TAG score sheet is the only page which needs to be completed for a TAG assessment. These pages are designed to answer most questions about the TAG.
